    This form exists for faculty wishing to refer a student to meet with the Writing Center personnel for a one-on-one tutoring session addressing specific issues and concerns with a particular paper assignment.

    This form will be submitted electronically to the Writing Center. We suggest that you provide the referred student a printed copy of the confirmation email to present to the Writing Center at the time of the appointment.

    Please note: This is not an appointment form. It is the student's responsibility, once you have referred them, to make an appointment with us.
